Parental Release Form:

I/We as parent/legal guardian(s) of the below named GOYAn, give permission for my/our child to participate in the Pan Orthodox Cross Dive (in Virginia Beach) of the Greek Orthodox Metropolis of New Jersey. This activity include swimming and diving. (Note: certified lifeguards supervise all water activities.) I/We understand that photos and/or video will be taken of the Cross Dive and Participants. Therefore, we consent to the use of any video images, photographs, audio recordings, or any other visual or audio reproduction that may be taken of my/our child during the Pan Orthodox Cross Dive.  I/We understand the possibility of unforeseen hazards and know the inherent possibility of risk in water activities. I/We agree not to hold the Cross Dive Committee, Greek Orthodox Archdiocese of America, the Greek Orthodox Metropolis of New Jersey, their leaders, employees, and volunteer staff liable for damages, losses, diseases, or injuries incurred by the subject of this form. Please note that in providing recreational activities, the Greek Orthodox Metropolis of New Jersey does not hold itself responsible in any way for any injury that might arise from participation in such events. Participation is strictly voluntary and carries with it the commonly held understanding that some degree of risk is associated with swimming/diving activities.
